Community High School District 218 has once again partnered with Mobile Care Chicago and Howard Brown Health to provide the Pfizer COVID vaccine to our communities at Delta/Summit Learning Center in Crestwood on July 14, 2021. At this time, we are offering 200 doses of the vaccine, which has been approved for persons 12 years of age or older. Please note that minors must be accompanied by a parent who will need to complete a consent form.
Follow-up vaccination (2nd dose) will be on August 4, 2021 at Delta/Summit Learning Center.
